NEXT STEP

Step 1:

If you are ready to take the next step forward, please fill out the Patient History Form.
A PDF file will open and you may then record the information which will allow us to set up an initial appointment.
 * Mac Users: Save  file and open in “Preview” to electronically fill out the form.

Step 2:

Complete the PHQ-9 checklist, a widely used quick measure of depression.

Step 3:

Please provide us with a complete list of all psychiatric medications you have taken. An accurate list is REQUIRED by most insurance carriers, or they will deny coverage for TMS.

If you have an online account with your pharmacy, this list is often accessible in your account settings. You can also call those pharmacies that you have used in the past ten years and ask them to print a list of their pharmacy records for you. However, don’t worry if your list isn’t complete; do your best and we will fill in any blank spots later on.

Final Step:

Once you’ve completed the Patient History Form, PHQ-9, and the list of psychiatric medications, please return a copy to our TMS Coordinator, Tricia Mandeville, via email, fax, mail, or in person.

At this time, she will also schedule you for your initial consultation with our
Clinical Services Director, Dr. Joe Hammock.
