Online Medical History Questionnaire Completed

I am pleased to announce that as of September 16, 2012, a very important component of the sctmsc.com website has been completed. It is now possible to arrive at our webpage NEXT STEP, and immediately download our medical history form. This document can then be completed at your computer, at your convenience, saved and e-mailed directly to our office.
As you may already know, TMS treatment is a medical procedure which requires an accurate and multi-page medical history be completed prior to our initial interview with each prospective TMS patient. Until that information is collected and then reviewed in our initial clinical interview, it is simply not possible to provide accurate estimates of length and cost of the procedure. Indeed, it’s not even possible to know if TMS would be an appropriate treatment until that information is available.

Now that the questionnaire can be downloaded, completed, and returned online, we are confident that it will be easier for everyone to get started.

This is excellent timing given the recent spate of good news about expanding insurance coverage for TMS treatments:

Joe Hammock, PhD, Clinical Services Director of Southern Colorado TMS Center, LLC, is a licensed psychologist and member of the American Psychological Association. He is a graduate of Fuller Graduate School of Psychology, and additionally holds an MA from Fuller Theological Seminary. He is certified by the International EMDR Association and by the Professional Academy of Custody Evaluators. Dr. Hammock works with adults, adolescents and couples. In his thirty years of practice in Colorado, he has specialized in the treatment of trauma-based disorders, depression, anxiety disorders, and dissociative disorders. He also has extensive experience in psychological testing and assessment, forensic psychology, and is frequently consulted as an expert witness. Adjunct to his professional practice, Dr. Hammock also specializes in assisting his clients to move beyond psychotherapy through the spiritual and personal growth principles of Awareness, Disclosure, Acceptance, and Discipline.
